Dear Volunteer,

 

Thank you for volunteering your time at Whittemore-Prescott Area School. We appreciate your willingness to be involved in the education of our community’s children.

 

We all agree that the children of the Community are precious and must be safeguarded at all cost. Toward protecting our children and maintaining a safe environment we are requiring all volunteers to undergo a criminal records check.

 

Please be aware that criminal history checks through the ICHAT system occasionally yield false results. In the event that you are not approved to volunteer, based on the ICHAT check, we will notify you of the results. If you believe the information from ICHAT is incorrect or inaccurate we will work with you toward getting the information corrected.

Volunteer Policy

 

All individuals interested in volunteering in any activity that will result in their working directly with students, in any capacity, shall undergo a criminal history check through the ICHAT system.

 

NO individual will be allowed to volunteer who has been convicted of a listed offense under the Michigan Sex Offenders Registration Act. In addition, any volunteer who has been convicted of a felony may not volunteer unless the Superintendent and Board specifically approve.

 

Volunteers who have criminal convictions less than a felony may volunteer only as approved by the Superintendent.

 

been convicted of, plead guilty or nolo contendere (no contest) to, or have any pending charges, to any crimes. (Other than minor traffic violations)

WAIVER OF APPLICANT: I understand that I may be required to provide fingerprints. I further understand and acknowledge that I am performing these hours of service for Whittemore-Prescott Area Schools without promise, expectation, or receipt of compensation for these services. I authorize Whittemore-Prescott Area Schools to make any inquiry or receive any information from any person or organization regarding my suitability as a volunteer and do hereby give permission to these persons or organizations to provide such information. For and in consideration of the release of such information, I hereby forever waive, release and covenant not to sue any such person or organization including Whittemore-Prescott Area Schools, its agents and employees for the result of providing, obtaining, or acting on such information. I give this release waiver, and covenant not to sue understanding that the information obtained may be such as to disqualify me from participation as a volunteer. I understand that such information is sought with confidentiality, and I will not request copies of such information. I also certify that all statements made on this application are true and complete, accurate and not misleading to the best of my knowledge. I understand that any false statements, incomplete statements, or misrepresentations may subject me to disqualification from participation as a volunteer.
